Eligibility to receive services
To receive services, a person needs to meet two criteria:
- No health insurance (no Medicare, Medicaid or private health insurance)
- Household income at or below 133% of the federal poverty level*
The 2012 federal poverty level is defined as
- $1,224/month – 1 person
- $1,639/month – 2 people
- $2,067/month – 3 people
- $2,494/month – 4 people
* – Individuals need to bring proof of household income (tax return, W-2 statement, pay stubs, earnings letter for social security or disability, UCB-104A form, IRS 4506-T form, most recent bank statement or letter from church, family, etc.)
Hours of Operation
Monday, Tuesday, Wednesday & Thursday (8 AM – 6:00 PM)
Friday (8 AM – 1:00 PM)
Saturday- Sunday (Closed)
Services provided:
- Primary care
- Medications & Diabetic supplies
- Patient Education
- Referral to laboratory, x-ray & diagnostic testing
- Referral to specialty care (24 specialties)
Services not provided:
- Pregnancy testing
- Abortions
- Birth control services
- Routine eye exams
- Immunizations
- Health care to pregnant women
- Physicals/second opinions
- Treatment for Sexually Transmitted Diseases
- Treatment for Hepatitis
